The Process Autopsy

Here is a messy description of a process that keeps breaking: [PASTE DESCRIPTION - WHO DOES WHAT, WHERE IT FAILS]. Map it: the actual steps in order, the owner of each step, the two or three failure points, and why each one fails (handoff gap, unclear owner, missing information, or bad tooling). Then propose the smallest fix for each failure point - process change before tooling change.
